A Baseball Tryout Evaluation Form is a feedback tool required to properly examine the abilities of a baseball player and determine if they are promising enough to be a valuable addition to the squad. This document can be prepared by the member of the coaching staff taking notes during the training session - similarly, you will have an opportunity to invite younger players from the academy or local residents who need a chance to prove themselves and earn a place in your team.
Once you have established the appropriate tests and drills, check the player's fitness, flexibility, core strength, speed and acceleration, agility, throwing velocity, hitting, and catching. Do not disregard the athlete's mental state - assess their body language, confidence, communication with teammates, reaction to failures, concentration, and patience.

Identify the player and draw a table listing all the offensive, defensive, and pitching skills you are reviewing in order to assess them. You should use a grading scale to indicate your opinion about the physical and psychological condition of the athlete instead of writing remarks that may later be misinterpreted. It is up to you to decide whether to rate the player by giving them points or describing their efforts with adjectives from excellent to poor. Alternatively, the evaluation can be conducted for several athletes at the same time - make sure you divide the sheet into several sections to avoid confusion when analyzing the tryout results.
